Body contouring isn’t always a matter of nipping, tucking and trimming. Often it’s enhancement, not reduction, that we’re looking for, and, whether it’s adding some volume to that visage, a boost to those breasts or a curve to your bottom line, cosmetic surgery has full body options.

Cleavage woes? If you long for a more shapely bosom than was granted by Mother Nature, or for restoring the lift and shape diminished by Father Time, look no further than your local board-certified surgeon.
Options for augmentation generally include saline and silicone implants, both approved by the FDA and widely considered to be highly safe. Saline implants are made of an outer silicone elastomer shell filled with sterile salt water and, because they can be filled after being inserted, only a small incision is usually needed. Silicone is actually a gel, and though it requires a larger incision to place, proponents say that silicone implants feel more like natural breasts.
In choosing between the two alternatives, then, various pros and cons must be considered. Again, the first consequence is where the incision will be made. Saline implants are first inserted then filled, which makes it possible to place them through incisions under the breast (inframammary), along the edge of the nipple (areola), in the armpit (transaxillary), or even through the navel (TUBA). On the other hand, silicone implants are pre-filled and may require a larger incision area, completely eliminating the bellybutton incision area from your list of choices.

Moving forward with a cosmetic surgery procedure, even a non-invasive one, is a major life decision that should never be taken lightly. The American Academy of Cosmetic Surgery is dedicated to patient safety and education, but in the end it is up to you, the patient, to make a wise choice based on your own research, understanding and level of comfort. Here are some solid guidelines to help you in making that decision.
What are the surgeon’s credentials? Don’t hesitate to verify a physician’s medical license, certifications and professional society memberships and ask what the requirements are to be a member or to be board-certified in that specialty.
What is the surgeon’s experience? Ask about the surgeon’s medical education and training specific to cosmetic surgery and the procedure that you are considering. How many procedures of this kind has the doctor done, and how often does he or she perform them?
Ask for referrals from friends. Keep in mind that physicians specialize in different areas of cosmetic surgery and most are not experts in every arena. The surgeon who did a fantastic face-lift on a friend may not be the most experienced to perform breast augmentation.
Before-and-after photographs can give you some indication of a surgeon’s ability, although you must realize that they cannot guarantee the result you will achieve. Each patient is different and your surgeon can help determine what kind of results you should expect.

He would like to share some of his thoughts about face lift surgery. A facelift is one of the top ten cosmetic procedures performed in the United States. Despite its frequent performance and its recognition by the public, it is a procedure that is usually misunderstood.
The most common misconception of a facelift is that it does very little for much of the face. It is a procedure that has its impact on the neck and jowl areas. The rest of the face is untouched by the operation. While other procedures are often done with it, such as eyes and brows, these procedures are not part of the traditional facelift operation. A facelift should more properly be called a necklift or a neck-jowl lift.
Another misconception about facelifts is what is actually lifted. Everyone understands that face and neck skin is lifted and moved back, but what goes on underneath the skin layer is a frequent point of misinterpretation. Patients often ask me if I am going to lift and tighten the 'muscles'. From an anatomic standpoint, lifting the facial muscles is an impossibility. Most facial muscles are attached tightly to the underlying bone and moving them would not only be very difficult but would not have any positive benefit. There is only one muscle that is often tightened in a facelift and that is the platysma muscle in the neck, which is often separated due to aging. It is never lifted per se but it is tightened in the middle, from the chin down to the adam's apple, to help sharpen the neck angle.
Fat grafting is a plastic surgery method whereby excess fat is taken from area of the body and then transferred to another in order to add plumpness. This is normally a relatively quick procedure of about two to four hours and done on an outpatient status.
The average patient that undergoes this procedure will have fat from their abdomen, thighs or buttocks transferred over to the area under the eyes, lips, temples or cheeks. Within just a few days, a person will be back to regular activities after undergoing this relatively simple procedure, when compared to other types of plastic surgery.

Before you have any type of plastic surgery performed on you, you must be sure to take the proper time to take a good look at the medical training and surgical experience of the doctor you end up with. For decades plastic surgeons have been looking to restore a more youthful look on the face. Surgical techniques have been improving; newer lasers have been used in order to achieve a goal that seemed unreachable. Comparing the post-operative results with the pictures of the patients when they were younger, it appeared that there was something missing: a younger face had more volume and the skin just looked fresher.

What is a Liquid Facelift?
A "liquid facelift" is a beguiling misnomer meant to deceive people who are reluctant to get surgery. The idea is that with injectible fillers such as Restylane or Captique and with dynamic wrinkle reducers such as Botox, you can achieve the same effects as a facelift. Now, let me present you with an analogy. Let's say that you needed to erect a tent that had a well worn canvas. The tent bows in the middle and sags in on the sides so that it minimizes the space you have to use. How would you fix it? Yes, you can stretch the fabric better and repair any holes but at some point, you have to get a new framework to stretch the tent over, because no matter how many adjustments you make to the canvas, the tent will not be useful unless you update the framework.
A liquid facelift is a temporary measure to make small repairs to the canvas. It is misguided to refer to it as a facelift because it will not restore the tone of sagging facial skin. The only way to do that appropriately is to perform a facelift. This procedure is accomplished by lifting deeper tissue under the skin and removing excess skin to restore the balance of a youthful face. The features one can expect with a facelift includes a well defined jawline and the absence of sagging neck skin. I am a strong believer in injectibles and they can be used to maintain a youthful appearance. I use Botox, Dysport, Juvederm and Radiesse in my practice daily. However, they have been oversold by those who don't perform surgical facelifts- often to keep their clients scared of surgery and insure that patients come back for an endless series of injections.

Platelet-Rich Plasma (PRP) Utilized To Promote Greater Graft Volume Retention in Autologous Fat Grafting

Objectives: Autologous fat theoretically provides one of the most ideal mediums for soft-tissue augmentation and reconstruction, although its clinical applications have been marked with skepticism because of its documented unreliable survival. Over the years, numerous unsuccessful efforts have set forth to elucidate modifications in the application process of autologous fat grafts to allow the medium greater clinical predictability. This study aims to investigate the effects of platelet-rich plasma (PRP) on autologous fat grafts when used in conjunction with each other in soft tissue augmen- tation and reconstruction.
Study Design: Retrospective review, over a 30-month period, of consecutive patients with results greater than 6 months in duration.
Methods: This study is based on clinical experiences representing 2033 grafts in 448 consecutive patients using PRP additives and in the previous 132 patients who had syringe harvest without use of PRP. All PRP isolates were harvested via the Smart Prep system. Harvest and augmen- tation techniques are discussed and representative results are presented.
Results: Results were based on clinical observations and patient satisfaction. Of the 580 patients in the experimental group, essentially all showed greater graft volume retention over extended time intervals compared with control subjects (nongraft areas). Patients in the PRP-added experimental group displayed less postoperative ecchymosis and edema, which also led to greater patient satisfaction in this group.
Conclusion: Adding PRP to autologous fat aids in graft volume retention and survival when used clinically for soft- tissue augmentation and reconstruction.
