Unlike many surgeons, Dr. Sadati takes time to get to know his patients. You will have an extensive consultation so that he can understand your goals for the rhinoplasty. Dr. Sadati will ask you to describe any physical or emotional trauma that is related to your nose. He will also need to know your medical history, including if you have problems breathing and have undergone previous nasal surgery. Nose is like a sport luxury car, it has to look good and at the same time perform well. A sport car that looks good but does not have a good engine is useless. Many plastic surgeon focus on making the nose look better while ignoring the importance of breathing function after the nose is even smaller and my become functionally dysfunctional after rhinoplasty.

Rhinoplasty Surgery in Orange County
Ideal surgical candidates are non-smokers who are in good physical and emotional health. Dr. Sadati will make specific recommendations about how you should prepare for your surgery. This may include stopping certain medications and supplements. Dr. Sadati will explain his surgical plan, including the length of the surgery and the type of anesthesia.
There are two types of rhinoplasty surgeries:
Most Orange County plastic surgeons (including Dr. Sadati) prefer the open rhinoplasty approach. By making a small incision under the nose and slightly lifting the skin over the nasal structure, the surgeon can view the underlying anatomy of the nose. This makes it easier to create the desired precise shape because the plastic surgeon can see the cartilage and bone being altered. Revision rhinoplasties are always performed using the open method.
The downside of an open rhinoplasty is that it takes slightly longer time than a closed rhinoplasty to perform. It also requires a more extensive surgical dissection of the nasal structures for precision framework, which may create prolonged swelling. To create stable structural support, the surgeon can add additional cartilage graft. Overall, an open rhinoplasty provides an excellent and stable result for patients.
Also known as an endonasal rhinoplasty is an older surgical technique, a closed rhinoplasty means that all the incisions are made inside the nose. While this sounds ideal, the reality is that a closed rhinoplasty is a very difficult procedure to perform because the surgeon cannot physically see the nasal anatomy. It is similar for a mechanic to fix the engine with out lifting the hood and fixing it by feeling under hood. A closed rhinoplasty take less time to perform than an open rhinoplasty and can have excellent results if performed by a specialized, highly trained plastic surgeon.
Physical Recovery Following Rhinoplasty
Depending on the difficulty of the rhinoplasty, your surgery will be performed using either a general anesthetic or IV sedation.
You will wake from surgery wearing a external nasal splint. This is required for recovery and must be worn for six days in order to protect your delicate nose. (Despite what you may have heard, we do not pack inside of your nose as a part of the recovery process. This is an antiquated technique that we have not used for more than ten years.)
You must have someone to drive you home and stay with you over night. If you prefer, you can stay in one of our luxurious guest suites, where one of our highly experienced nurses will monitor you over night. Although rhinoplasty is not a painful procedure with only mild discomfort, just in case you will be prescribed pain medication to keep you comfortable.
We recommend using cold packs for the first 48 hours following surgery. Frozen peas or crushed ice in a zip-lock bag work well. This will help reduce swelling, bruising and pain. We recommend that you apply ice every few hours, for no longer than 20 minutes. Place a cloth between your nose and the ice pack to protect your skin. Dr. Sadati will give you information on your post-operative diet. Generally we recommend starting with bland, soft foods and then transitioning to your regular low salt diet. If you experience nausea following your surgery, we recommend soft foods such as mashed potatoes or pudding, or using crackers to help settle your stomach. We will prescribe Zofran if you have severe nausea.
The average recovery for a rhinoplasty is four to eight days. Post-operative healing depends on the complexity of the surgery and how well you follow your post-operative instructions. We strongly recommend that you avoid any strenuous activity during your recovery so that you don’t cause additional bleeding and swelling. Allow yourself time to rest and recover from surgery!
The interior function of nose is to clean and humidify the air for lungs. Since the interior of your nose will continue to be swollen for several weeks, you need to use ocean nasal spray to moisturize it. At that time, your airway will start to clear and you will be able to breathe clean and moisturized air. Your sleeping pattern will also improve.
You must not drink alcohol during your rhinoplasty recovery. Alcohol dilates the blood vessels and can increase post-operative bleeding. Combining alcohol and pain medication is dangerous. We also ask you not drive while you are taking prescription pain medication.
Do not smoke or use cigarette replacements during the recovery process. Smoking will decrease the oxygen supply, which can inhibit wound healing and cause tissue death.
Emotional Recovery Following Rhinoplasty
In the days after surgery, some patients question their decision to undergo rhinoplasty. This is not unusual.
It can be traumatic to experience the incisions and swelling that occur following rhinoplasty. If you’re feeling overwhelmed, remember that you are in the early phases of recovery and this feeling will pass. Second-guessing your surgery is normal. These thoughts generally disappear within a few days as the healing occurs. Remember that you’ve entrusted your nose to a highly skilled plastic surgeon with many years of experience. After the incisions heal and the swelling subsides, you will feel wonderful!
It is human nature to compare ourselves to others and to solicit feedback on our progress. Unfortunately, the input of family and friends can create emotional turmoil during your recovery.
